2/22/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Khanilya appeared to be in a content mood. Khanilya was upset at the onset of the session as she verbalized having had an argument with her mother prior to QMHP-C arriving to the home as she was telling her mother she was not going to clean up. Mother verbalized she really donât know what is getting into her when she tells her do complete certain task within the home. Khanilyaâs mother came into the room and began to speak over Khanilya explaining what occurred during the incident and eventually admitted being tired of Khanilyaâs lack of respect for her as a mother as Khanilya verbalized to her mother crying as well as QMHP-C that her brother was the one who made the mess and needed to clean up. 2. Behavior: Khanilya participated in the session without incident Khanilya discussed with QMHP-C she deals with a lot of frustration. Khanilya discussed on how she continues to control her anger but others in the home makes her upset which causes her and her mother to get into it and as a result she started yelling throughout the session. 1. Intervention: QMHP-C engaged Khanilya in a one on one individual session with Khanilya to address her displays of aggression and defiance towards authority and to educate on the consequences that will continue to occur until Khanilya complies with rules. QMHP-C assisted Khanilya in identifying possible triggers of anger and situations to avoid. QMHP-C addressed the importance of remembering to pick her battles with authority while keeping in mind situations that are a win-lose situation for her. 2. Intervention: QMHP-C assisted with identifying times in the past when she used positive coping strategies to solve problems and revisited the positive outcomes that should encourage her to use them again when she is faced with similar problems. QMHP-C processed with Khanilya additional coping strategies she could use in the future to deal with stress or frustration more effectively without the use of verbally aggressive language that only escalate the conflict. QMHP-C challenged Khanilyaâs way of thinking throughout the session and facilitated role play and role reversal for understanding of skills taught. 1. Response: Khanilya was receptive to QMHP-Csâ intervention. Khanilya was complaint with this session as she acknowledged her disrespect towards her mother who she felt was targeting her all the time for conflict. Khanilya was able to identify her role in the conflict and verbalized the need to make some changes. Khanilya was verbal in identifying the things people do to trigger her anger but had difficulty accepting her role in some of those past conflicts as she feels others tend to trigger her anger purposely. Khanilya acknowledged how effective it is when she takes the time to identify the feelings and changes, she believes she goes through before becoming angered and attempting to de-escalate herself. 2. Response: Khanilya was able to identify the areas she needs to continue to focus on mastering as she works towards improving her relationship with her mother as she agreed she would like to see them get along better and bond more. Khanilya began to display an understanding of coping a skill learned to day and was able to express ways to apply when needed to make better choices when feeling stressed or overwhelmed. Khanilya agreed to understanding the importance in medication compliance and verbalized consistency without requiring guidance.
